You mix things up.
When you do boolean operations, you are using Nef_polyhedron_3 I guess.
This class must be used with an exact number type.
Then, you are using a Polyhedron_3 with, I guess, a floating point number type. So there is a conversion which done that might introduce
the self-intersections.
The edge simplification algorithm cannot work correctly if there are self-intersections.

let the function 'SimplifySurfaceMeshCGAL' read OFF file. My work is to
create cylinders and boxs (of cause they are all polyhedrons) and then let
the CGAL to do boolean operations to combine all these objects to the final
one. The funcitons 'SimplifySurfaceMeshCGAL' directly works on the final
polyhedron. My questions are:
1) for the raw OFF file (a1.off) outputted by CGAL, what could lead to
'self-intersection' and duplicated vertices on some triangles?
2) why the function 'SimplifySurfaceMeshCGAL' still can't remove the
duplicated vertices (with edge length< 1.e-14) when the data is directly
from boolean operations result?
